FYI- I became deeply interested in the much maligned "germ theory" of cancer 30 years ago and wrote a few chapters on one of the now "discredited" pioneers of that theory, Virginia Livingston. I enjoyed reading Kell's paper. I think I understand it well enough to comment.
I dont think his model can account for bartonella.
regarding point 3
"...by microbes that become and remain dormant, largely because they lack the free iron necessary to replicate..."
This probably doesnt apply to bartonella because bartonella almost exclusively acquires iron from heme-iron. Bartonella lacks the classic membrane proteins that transport free-iron into it's cell.
